Harmony House

Support for day to day living in the Community program - Day 2 Day Living Programme
The Campbelltown "Harmony House" Support for Day to Day Living in the Community Program (D2DL) is one of the many services provided by the Schizophrenia Fellowship of NSW, and is funded by the Department of Health and Ageing (DOHA).
The D2DL program aims to assist people with a severe and persistent mental illness on their journey to recovery. It assist people experiencing social isolation in the community.
D2DL Project Aims and Objectives:
Development of day-to-day living skills by learning new skills or relearning old ones
Strengthening social networks
Increasing participation in community, social, recreational and educational activities
Improvement in self confidence
.
Achievement of personal goals and the ability to participate in everyday living activities
Improved quality of life
Supporting people to live at an optimal level of independence in the community

Referral and Intake:      
Campbelltown " Harmony House" D2DL program will accept referrals for people with severe and persistent mental illness who experience isolation in the community.
The intake process will assess a participant's needs and access to relative support in terms of:
 - Physical Needs: food, clothing, shelter, safety and income
- Mental Health Needs: these will be addressed by engaging a collaboration of services including Mental Health Teams, NGOs, GPs and other appropriate services in the community.
- Psycho-Social Needs: This will include exploring areas of self of self confidence, knowledge and skill development, in respect to social, recreational and educational well being.
 
Individual Recovery Plan:
Participants will be encouraged to actively engage in their recovery process, by developing Individual Recovery Plans with staff support at the Campbelltown " Harmony House" D2DL program. This process is designed to identity the individual's goals and create productive steps to achieve them.
